Resident Form and Submission

September 1 is the deadline to apply to this program.

We strongly recommend all applicants to apply early and have a complete application on file with ADEA PASS prior to the deadline.

Step 1. Apply to PASS

All program applicants must apply through PASS (Postdoctoral Application Support Service), which is sponsored by the American Dental Education Association (www.adea.org). PASS is an application clearinghouse that provides uniform information to participating programs while relieving applicants of the burden of completing multiple individual applications. The PASS website opens for application in the early summer (May). Participation in the PASS program is mandatory. We advise applicants to apply as early as possible. Please check the ADEA PASS Program Finder for any updates by April 1.

Additional documentation to include on PASS application:

  • Resumé no more than (a two-page resume is strongly suggested)
  • 300-500 word Personal Statement stating your interest in the program.
  • Two (2) Personal Potential Index Evaluation Forms (PPIs) completed on ADEA PASS. Please note: an Institution Evaluation Form (IEF) does not equal a PEF
  • Copy of course-by-course and GPA evaluation report by a certified translation service.

Step 2. Applicants must mail directly the following documents to the program:

  • Applicants should Official National Boards Part I and II or Integrated National Board Dental Examination Score Report to be emailed directly to raurelus@csmiles.org  

For foreign trained dentist, please mail the following:

  • Color Copy Official Dental School Transcript and Certified English translation of Official Dental School Transcript by certified translation service.
  • Color Copy Official Dental School Diploma (DMD/DDS/BDS Degree) for every institution of higher learning which you have attended and Certified English translation of Official Dental School Diploma by certified translation service.
  • Official Course by Course and GPA Evaluation directly from ECE or WES in an envelope sealed by the evaluation service. Educational Credential Evaluators, Inc., (ECE) P.O.   • Official three (3) current letters of professional references from a dental school dean, dental faculty, dental department chairs, dental directors, supervise faculty or dentists who have supervised your work.
  • Color Copy of Permanent Resident Card or Certified of Naturalization or F1 Visa holder with Social Security Number. (If you are accepted in the program. It is the student’s responsibility to maintain their visa for the duration of the program.)
  • Resumé and Application Checklist (please provide this cover letter and submit all documentation in the order being requested).
  • Passport Photo (a total of two (2) is required).
  • A non-refundable $250.00 application fee (Cashier’s Check, Money Order or Personal Check) Pay of Order to: Community Smiles Dental Clinic
  • Official TOEFL Essential Score Report (passing score of 80 or higher on web-based exam) School Code: 2285 (For the TOEFL scores expire after 2 years. For the Application Cycle, TOEFL scores cannot expire from year dated application)

Translations must be sent to Dade County Dental Research Clinic d/b/a Florida Institute for Advanced Dental Education in an envelope sealed by the translation service.

Enrollment Requirements

Upon enrollment into Dade County Dental Research Clinic d/b/a Florida Institute for Advanced Dental Education (AEGD) Programs, students must provide

  • Copies of B.D.S., D.D.S. or D.M.D. degrees awarded
  • Final dental school transcripts (for pending dental degree applicants)
  • Copies of diplomas or certificates awarded from advanced dental education and dental specialty programs
  • Proof of current Basic Life Support certification
  • Copies of all current state dental licenses
  • Verification of any malpractice proceedings brought against them (together with all other appropriate documentation)
  • Must be a United States citizen or have a permanent resident visa (green card) or F1 Visa holder with United States Social Security Number and proof of your status.
  • Must have successfully completed the National Board Dental Examination Part I or Integrated National Board Dental Examination. Visit www.ada.org  Successful completion of National Board Examination Part II is not required for admission, but must be successfully completed prior to the commencement of the second year of the AEGD program.
  • This program requires two (2) academic years to complete to come a dentist in the State of Florida.
Interview Process
  • Competitive applicants will be invited to a personal interview at Dade County Dental Research Clinic d/b/a Florida Institute for Advanced Dental Education by the Selection Committee.
  • The following official documents must be presented the day of interview:
    1. Original dental school diploma
    2. Original dental school transcripts
    3. Original proof of citizenship (US passport, US birth certificate, or Certificate of Naturalization) or proof of permanent resident visa (green card) or proof of F1 Visa holder
    4. Original Social Security Card
  • Interviews for the Dade County Dental Research Clinic d/b/a Florida Institute for Advanced Dental Education AEGD Program will be held during the months of September or October.
Application Status
  • The Office of Admissions will work hard to process your PASS application in a timely manner. Please be aware that it can take up to four weeks.
  • Within two weeks of receiving your PASS application, the Office of Admissions will send you a confirmation via email.
  • After processing your PASS application, which can take an additional two weeks, the Office of Admissions will send you an email outlining your current application status and any remaining actions you need to take in order to complete your application.
  • Complete applications will be forwarded to the program director on a rolling basis.
  • Applicants with incomplete applications will not be eligible for admission.
Bench Exam
  • Selected candidates are required to participate in the bench exam at the Dade County Dental Research Clinic d/b/a Florida Institute for Advanced Dental Education in Miami, Florida.
  1. Practical Exam: This examination will consist of a typodont-based examination where the applicant is required to show acceptable skills in basic clinical procedures in operative and fixed prosthetics. Restorative procedures must be performed without identifiable critical errors.
  • Applicants who are invited to interview will be notified via email.
Medical Requirements/Immunizations

Prior to enrollment, all advanced education students are required to have documentation of immunity to measles, mumps, hepatitis B, tetanus/diphtheria/pertussis and rubella, COVID-19, and to have an tuberculosis skin test. All advanced education students shall be required to comply with the infection control policies and procedures of the facilities where their rotation occurs. Measles, Mumps, Hepatitis B, Tetanus/Diphtheria/Pertusis and Rebulla must within two years and Tuberculosis Skin Test. Any approved inoculation for healthcare professionals by the CDC and the State of Florida.
